Minnesota Secures $193 Million to Transform Rural Health Care

The Minnesota Department of Health has received a substantial investment of more than $193 million for rural health care as part of the inaugural year of the Rural Health Transformation Program (RHTP). This funding, announced by the Centers for Medicare & Medicaid Services (CMS), is designed to enhance health care delivery in rural areas across the state. The RHTP aims to stabilize and improve access to health services for communities that often face significant challenges.

The funding allocation includes both base payments distributed to approved states and additional resources that highlight Minnesota’s innovative approach. By 2026, all 50 states are set to receive first-year awards under this program. The investment will support several key initiatives aimed at bringing care closer to home for rural residents, improving health outcomes for individuals at risk of chronic conditions such as cardiovascular disease, diabetes, and chronic kidney disease.

The funding will facilitate strategic investments in new technology that enhances care delivery. Initiatives are expected to focus on improving access through mobile and community-based services, strengthening partnerships among providers, and ensuring the financial stability of rural health care providers through targeted investments in technology and data infrastructure.

Minnesota’s proposal was crafted through extensive collaboration with various stakeholders, including rural community organizations, hospitals, health systems, safety net clinics, tribal authorities, and local public health agencies. The application, submitted on November 4, requested $1 billion over five years, averaging $200 million per year. The development of this plan involved input from over 40 meetings with stakeholders and nearly 350 public responses, signaling a strong community interest in effective rural health solutions.

Future Engagement and Program Scope

The RHTP is part of a broader initiative established under H.R.1 (Public Law 119-21), which allocates up to $50 billion nationwide over five years to support innovative rural health initiatives. Minnesota’s health officials are poised to continue engaging stakeholders as the implementation process begins in early 2026.
