Massachusetts Reports First Silicosis Case Linked to Countertop Stone

Massachusetts health officials have confirmed the state’s first case of silicosis, a serious and incurable lung disease, in a 40-year-old man who spent 14 years working in the stone countertop industry. This case underscores the risks associated with exposure to crystalline silica dust, particularly from engineered stones such as quartz, which has gained popularity for its aesthetic appeal and practicality.

On October 3, 2023, the Massachusetts Department of Public Health (DPH) revealed that the patient, who has not been identified, was diagnosed with silicosis after engaging in activities like cutting, grinding, and polishing stone countertops. These processes can generate fine crystalline silica dust, which, when inhaled, causes scarring of lung tissue, leading to this debilitating disease.

Emily H. Sparer-Fine, a director at DPH, emphasized the gravity of this case, stating, “The confirmation of this case in Massachusetts is a tragic reminder that silicosis is not just a distant threat. It is here, and it is seriously impacting the health of workers in Massachusetts.”

Silicosis is a progressive condition that can lead to severe complications, including lung cancer, tuberculosis, and even death. Symptoms typically manifest as a persistent cough, shortness of breath, fatigue, and chest pain. Due to the long latency period between exposure and the onset of symptoms, many cases are diagnosed only after significant damage has occurred to the lungs.

The risk of developing silicosis is particularly high among workers handling engineered stone, which can contain over 90% silica. In contrast, natural stones like granite generally have lower silica content, typically below 45%. The DPH noted that while this is the first confirmed case in Massachusetts, it is unlikely to be an isolated incident given the long latency associated with the disease and the increasing use of engineered stone in the industry.

Recent studies reveal that silicosis cases are on the rise. A study conducted in California identified 52 workers involved with quartz countertops who had developed silicosis; of them, 20 had advanced disease and 10 succumbed to its effects.

While the potential dangers of quartz countertops are becoming more recognized, no outright ban on their use currently exists in the United States. In sharp contrast, Australia has already implemented a ban on all work involving engineered stone due to the heightened risks to worker health. Other countries are also advocating for stricter regulations to protect workers from these hazards.

Robbie Goldstein, the Public Health Commissioner, stated, “Silicosis is a devastating, life-altering disease and one that is also absolutely preventable.” He urged employers in the stone countertop fabrication industry to adopt robust safety measures, including wet cutting techniques and proper ventilation systems, to mitigate silica exposure and safeguard their employees.

As awareness of silicosis grows, health officials continue to stress the importance of preventive measures. The DPH aims to educate employers and workers on the risks associated with silica exposure and the necessary precautions to take. The call to action emphasizes that effective workplace controls can significantly reduce the incidence of this preventable disease.

With the rise in engineered stone’s popularity, the health implications for workers in the stone countertop industry must remain a priority. As this case demonstrates, silicosis poses a real and immediate threat to those exposed to silica dust, calling for urgent attention and action from both employers and regulatory authorities.
